1. We respect your privacy
We have a simple approach to data protection and privacy which adheres to the 13 Australian Privacy Principles.
Our obligations under the Privacy Act 1988 (Privacy Act):
- This privacy policy sets out how we comply with our obligations under the Privacy Act. As an Australian based organisation, we are bound by the Australian Privacy Principles (APPs) in the Privacy Act which regulate how organisations may collect, use, disclose and store personal information, and how individuals may access and correct personal information held about them.
- In this privacy policy, ‘personal information’ has the same meaning as defined by section 6 of the Privacy Act: information or an opinion (including information or an opinion forming part of a database), whether true or not, and whether recorded in a material form or not, about an individual whose identity is apparent, or can reasonably be ascertained, from the information or opinion.
2. Collection of personal information
Clean Kweens will receive and store personal information you enter onto our website, enter into Halaxy when booking an appointment or provide directly to us during your consultation. We may also receive personal information in other forms or from third parties, in which case we will protect it as set out in this Privacy Policy.
When booking an appointment via Halaxy or through the website, we collect the following information:
Name
Gender
Date of birth
Contact information (including email, phone number, address)
Medical history
Credit card details to secure your booking
When engaging in a consultation with one of our Dietitians, whether via Telehealth or face-to-face, we will also be collecting personal information about your diet, health and lifestyle choices. This enables us to conduct an in-depth nutrition assessment and provide appropriate education and advice that is individualised and will help you achieve your goals. We may also collect any other information you provide while interacting with us, whether it by via appointment, phone, email, social media, our website or other means.
3. Use of your personal information
We collect and require your personal information to understand your health status, needs and nutrition goals, allowing us to provide you with a better service. We require your personal information for administrative business purposes, including but not limited to:
Internal record keeping, documentation and auditing purposes
Information to improve our product and service
Send reminder, educational or promotional emails that are relevant to you
Your personal information may be used to correspond with other health professionals involved in your care, including doctors, specialists, psychologists or other health professionals. This assists in ensuring a holistic team approach to your healthcare and may be necessary to keep other health care professionals up to date with your progress or health status.
We may disclose your personal information to any of our employees, insurers, professional advisers or suppliers as reasonably necessary for the purposes set out in this Policy. Personal information is only supplied to a third party when required for the best practice of our service.
We will not sell, distribute or lease your personal information to third parties unless we have your permission or are required by law to do so.
4. Security of personal information
Your personal information is stored in a manner that protects it from misuse and loss and from unauthorised access, modification or disclosure. Suitable physical, electronic and managerial procedures have been put in place to ensure this security.
Where your personal information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or de-identify it. However, most personal information is or will be stored in our client files for a minimum of 7 years.
5. Access to your personal information
You may access the personal information we hold about you in accordance with the provisions of the Privacy Act 1988 (Cth). This may be to update or correct the information. If you wish to do this, please contact us at directly.
We reserve the right to refuse to provide you with information that we hold about you in certain circumstances set out in the Privacy Act.
You may request to decline correspondence with anyone in your healthcare team, such as a doctor or allied health professional, at anytime. However, please note that as health professionals we have a duty of care to report any life threatening risks to your doctor, regardless of your consent.
6. What we collect and receive from others
- Online Activity
cleankweens.com uses cookies and other digital identifiers that fall into the following 3 types:
- Site performance indicators: these give us information on how our websites are used, so that we can make the necessary improvements or amendments to make your experience a better one.
- Analytics Cookies: these give us the statistics to understand how many people are on our website, which areas are popular and which areas we need to work on.
- Advertising Cookies: we use these cookies to understand what type of advertising would be better suited to someone like you. That way, you would receive ads that are more likely to be relevant to you.
Cookie rights: You can always disable the use of cookies by changing the security settings of your browser. Just bear in mind that this may affect how some items are displayed on our website.
- Social Network widgets and buttons (Instagram, Pinterest, Twitter etc)
By clicking or using any social network widgets on our website, you understand that you do so according to the terms and privacy policy of those services. ‘Liking’ or ‘Following’, and content using these widgets, may become visible to your social network.
- Maps/Video and other embedded content
Embedded content such as Google Maps, YouTube videos etc. is embedded or placed on our website – it is not hosted on our servers and therefore any interaction with such services is done so according to their privacy policy.
7. Changes to our Privacy Policy
Please be aware that we may change or update this Privacy Policy in future. It is currently effective from 1st March 2022.
